How do I put custom information into User Defined Fields on the Order Header?

The Checkout.aspx page has 5 TextBox controls txtUser_def_fld_1 - txtUser_def_fld_5.  These are hidden by default.  To add put information in them, simply unhide them and add a label that describes the information you want.
